This form is to order replacement badges/certificates or additional badges post examination for completed Speech NZ qualifications.

All badges cost $12.00 each. If you are ordering 1-3 badges, postage will be $1.50. 4 or more badges will require a courier, for which the charge is $6.50 per order.

Payment: An invoice will be sent to you within the next few days. Please make payment using the information listed on the invoice. Orders will be sent once payment is received.
